El formulario firmado debe incluir la documentaci\u00f3n pertinente a la solicitud de inactivaci\u00f3n; QUAD A podr\u00e1 solicitar informaci\u00f3n adicional seg\u00fan las circunstancias.<\/p><p>QUAD A reembolsar\u00e1 el 50 % de la cuota anual y el 100 % de la cuota de inspecci\u00f3n, siempre que la solicitud o el paquete de renovaci\u00f3n se hayan presentado y procesado, pero la instalaci\u00f3n no haya sido inspeccionada. Si QUAD A ya realiz\u00f3 la inspecci\u00f3n, solo reembolsar\u00e1 el 50 % de la cuota anual y no la cuota de inspecci\u00f3n. Se descontar\u00e1 una tarifa administrativa de $100 de cada reembolso. Si la instalaci\u00f3n no completa el proceso de acreditaci\u00f3n dentro del plazo de un a\u00f1o a partir de la fecha en que QUAD A recibi\u00f3 la solicitud, esta se considerar\u00e1 vencida y la instalaci\u00f3n deber\u00e1 presentar una nueva solicitud y abonar las tarifas correspondientes para obtener la acreditaci\u00f3n.<\/p><p>Para renovaciones, si la instalaci\u00f3n no completa el proceso de acreditaci\u00f3n dentro de los 6 meses posteriores a la fecha de la factura, la acreditaci\u00f3n ser\u00e1 revocada.
