As of October 13, 2020, at 11am Central Time, many facilities were still non-compliant with Quad A\u2019s 2020 Period 3 PSDR requirements. If you work with multiple Quad A accredited facilities, please sign into the PSDR system<\/a> and check on your compliant status. If you have come into compliance (including submission of your facility\u2019s cases and any applicable physician override forms) after October 13, 2020, at 11am Central Time then please ignore this reminder. <\/span>Please note that any staffing changes that occurred during the period may have an impact on your status and you need to communicate with our office to ensure that we are aware of such changes.<\/span><\/i> <\/span><\/p>\n\n\n\n PSDR Period 3 relates to cases performed in an Quad A accredited facility between July 1 and September 30.\u00a0 <\/span>Random\u00a0case reviews for\u00a0PSDF\u00a0Period 3 are due\u00a0by midnight October\u00a018, 2020<\/span><\/strong> (Quad A has provided a\u00a0three-day\u00a0extension over the weekend to encourage compliance). Please make sure your facility\u2019s physician(s)\u00a0each\u00a0submit 3 random cases and any sequelae that occurred during the period.\u00a0<\/span>If any physicians performed fewer than 3 cases during the period, please submit data for all cases such physician completed and\u00a0send a\u00a0completed\u00a0Physician <\/a>Exemption Form<\/a> to the Quad A office.<\/span><\/i>\u00a0<\/span><\/p>\n\n\n\n Quad A will place facilities that remain non-compliant with <\/span><\/strong>Period 3 <\/span><\/strong>PSDR<\/span><\/strong> reporting on probation as of <\/span><\/strong>October 1<\/span><\/strong>9<\/span><\/strong>, 2020<\/span><\/strong>. Facilities that are placed on probation have 60 calendar days <\/span>to <\/span>submit their cases through the online <\/span>PSDR<\/span> system<\/span> and will be charged a penalty fee of $100 per non-compliant <\/span>provider<\/span>.<\/span> <\/span>Failure to comply with this requirement by the end of the probation term may result in revocation of the facility\u2019s <\/span>a<\/span>ccreditation.<\/span> <\/span><\/p>\n\n\n\n Please contact the Quad A PSDR Help Desk directly with questions or concerns:
