What to Know: Technical Corrections to Updated QUAD A Standards

QUAD A has issued a new Technical Corrections Document, effective April 7, 2025. These updates take precedence over previous standards, guidance in past newsletters and individual responses to standards-related questions.

These technical corrections clarify, revise, or eliminate previous requirements, keeping our standards aligned with current best practices and operational realities. Whether your facility is in the process of becoming accredited or maintaining existing accreditation, reviewing and implementing these updates is vital to remain in compliance.

Highlights of the Changes Include:

  • Anesthesia Administration Clarified: Class A sedation may now be administered by a registered nurse, nurse practitioner, or physician assistant under the direct supervision of a credentialed physician.
  • TIVA Guidance Updated: Facilities using Total Intravenous Anesthesia (TIVA) with no inhalation anesthetics are not required to have an anesthesia machine (see Standard 4-C-18).
  • Standard Exemptions for Class A Facilities: Several standards do not apply to Class A facilities.
  • Credentialing Timelines Extended: Primary source verification and re-credentialing are now extended to every three years instead of every two years (Standards 11-C-11 and 11-C-17).
  • Sample Size for Case Review Set: For Standard 10-B-6, the minimum quarterly review is now defined as 10% of your average monthly case volume.
  • International Clarification Added: A note to Standard 7-C-4 states that FDA reprocessing rules do not apply to international facilities—International facilities must comply with their local, provincial, or federal requirements.
  • Specific Updates for RHC and OPT Facilities: Certain standards have been excluded or revised for applicability to Rural Health Clinics and Outpatient Physical Therapy facilities.
  • Additional updates for clarity and consistency.
